One dose is required ten years after last dose of DTP/DTaP/DT
Four doses of polio vaccine one of which must have been received on or after the fourth birthday; however, 3 doses meet the requirement if the 3rd
dose was given on or after the 4th birthday
Two doses of a measles-containing vaccine with the first dose on or after
the first birthday; second dose required upon entry into kindergarten
